What is the Blue Note Fund?
The Blue Note Fund is a financial assistance program that provides one-time grants of $300 to colon cancer patients in need. At this time, awards will only be given to patients in active treatment. Grants will be awarded based on need.
How can I apply to the Blue Note Fund?
To apply for the Blue Note Fund, please print and fill out a Blue Note Fund application. Completed applications may be mailed to the CCA:

What year do I need to submit for my tax returns to qualify?
For the 2012 1st Quarter (January-April) please submit your 2010 tax return. After April 15, 2012, we will only be accepting 2011 tax returns.
What if I don't file taxes because I have no income or am on Social Security or disability?
If you do not file taxes, the following documents will qualify as proof of income: government year-end report for disability benefits, a bank statement showing how much has been deposited over the year, or a copy of your government check to calculate income. If you have no reportable income or supporting documentation, the CCA will accept a signed letter from the oncologist or doctor’s office stating that there is no income for the entire household including no patient income. This should be on official office stationery and be the same doctor’s office that is listed on the application.
If I'm a survivor, but not currently in treatment, do I still qualify?
Due to limited funds, we are only accepting applications from patients who are in active treatment at this time.
When will I be notified?
All applicants will be contacted with a notification letter once the review process is complete. The volunteer review committee meets quarterly and notification letters will be mailed the weeks of May 1, August 1, November 1 and February 1.
What is the cut-off date to submit an application for the April, July, October and January notification dates?
You must submit your application 15 days prior to the dating dates of May 1, August 1, November 1 and February 1.
Is the Blue Note Fund available to persons outside of the United States?
At this time, we are only able to award Blue Note Fund grants to United States residents.
