What is the Blue Note Fund?
The Blue Note Fund is a financial assistance program that provides one-time grants of $300 to colon cancer patients in need. At this time, awards will only be given to patients in active treatment. Grants will be awarded based on need.
How will my donation make a difference?
Your generous donation to the Blue Note Fund provides hope to patients and their families who desperately need it. 80% of your gift goes directly to Blue Note Fund recipients, and the other 20% supports the administrative costs that make this program possible.
How can I apply to the Blue Note Fund?
To apply for the Blue Note Fund, click here to fill out our online application.
Beginning October 16, 2012, all Blue Note Fund applications must be completed online. We will not be accepting paper applications through mail or fax. If you have any questions please contact the Blue Note Fund team at bluenotefund@ccalliance.org.
Do I need to submit any proof of income with my online Blue Note Fund application?
You do not need to submit any proof of income along with your application. However, from those chosen to receive a Blue Note Fund award, we will randomly select several applicants to send in their proof of income. You MUST be able to provide proof of income upon request.
If I'm a survivor, but not currently in treatment, do I still qualify?
Due to limited funds, we are only accepting applications from patients who are in active treatment at this time.
When will I be notified?
If you are selected as a Blue Note Fund award recipient, your award will be sent in the mail shortly after the review process is complete. All applicants not chosen to receive an award will be notified via email. The review committee meets quarterly and applicants will be notified in January, April, July, and October.
What is the cut-off date to submit an application for the January, April, July, and October notification dates?
The quarter deadlines are March 31, June 30, September 30, and December 31.
Is the Blue Note Fund available to persons outside of the United States?
At this time, we are only able to award Blue Note Fund grants to United States residents.
